PHYSICAL EXAMINATIONS

Physical exam forms are required for all students who are new to our district and grades Pre-K, K, 2,4,7,and 10. Camp physicals will satisfy this requirement. The forms are given out at the time of registration and in the spring (sent home with your child) and must be submitted within 30 days after entrance into the school or grade as mandated by New York State.

STATE MANDATED IMMUNIZATION

Immunization Changes for the School Year

Under section 2164 of the New York State Public Health Law, all children attending a school, day care center, or any preschool program must be immunized against Diphtheria, Polio, Measles, Mumps, Rubella, Haemophilus Influenza b, and Hepatitis b. Children who attend a preschool or day care program must show evidence of Lead Screening. 

 Minimum requirements include: 

• DTP, DTaP, or Td: 3 doses 
• IPV, OPV OR EIPV: 3 doses 
• MMR (Measles, Mumps, Rubella): 2 doses
• ADMINISTERED AFTER 12 MONTHS OF AGE, or show certification of disease 
• HBV (Hepatitis b): 3 doses 
• Varicella 1 dose children born on or after 1/1/94 
• Pneumococcal Vaccine (PPV) 
• Prevnar 

A Health Services Registration Card should be on file in the Health Office. This form shows all required immunizations and a Health History of your child.

Students will not be permitted to attend gym or outdoor recess until a current physical examination is on file in the Nurse's Office.

ALL DATES (month, day, year) ARE REQUIRED. Your child may not attend school without this information.

In the interest of all the children at Lakeville School, First Aid and Communication with parents takes first priority. Throughout the school day, informative phone calls will be made if needed. To contact the school nurse during the school day, telephone numbers are:

When reporting an absence, the telephone number listed above is an answering machine available for your convenience at all times. Upon your child's return to school, please send an absence note to the teacher. Children are expected to arrive at their classroom between 9:05-9:15. Any student arriving after that time must report to the health office to receive a late pass. In the event of an unreported lateness, parents will receive an automated phone call notifying of their child's absence.

Emergency cards are distributed at the start of the year to ensure adequate communication and safety while your child is at school. It is essential that these cards have updated telephone numbers at all times.

Throughout the school year, VISION, HEARING and SCOLIOSIS screenings are done
while your child attends school. It is important to remember, the school is not a diagnostic facility.
Should there be concerns for further evaluations, a letter will be sent to you to seek an evaluation by your child's private physician.

If medication is to be administered to your child during the school day, New York State Education Department requires: 

• A written order from the doctor 
• A written note from the parent 
• The medication in a properly labeled pharmacy bottle
  • The medication must be delivered to the Health Office by a parent or an adult designated by the parent (This requirement refers to over-the-counter medications as well)
